At HarmonyCares Hospice, we provide patient and family-centered hospice and palliative care services. Our care team focuses the patient's needs in their home, home of a loved one, or in a long term care or assisted living facility. We are proudly accredited by the Community Health Accreditation Partners (CHAP).
The goal of hospice care is for the patient to remain as comfortable and pain-free as possible in their own home or the home of a loved one through the end of life.
